Shore Sight Publication Scheme

Welcome to the publication scheme for Shore Sight Opticians, which is required by the Freedom of Information Act 2000.

This publication scheme is a complete guide to the information routinely made available to the public by Shore Sight Opticians. It is a description of the information about our NHS services which we make publicly available.

How much does it cost?

The publications are free unless otherwise indicated. Where information is provided at a cost, the charges will be calculated as set out in Class 7 below.

Your rights to information?

* The aim of the Freedom of Information Act 2000 is to provide members of the public with access to information held by public authorities.

* You are entitled to certain information in relation to the NHS services we provide under the Freedom of Information Act 2000 and the NHS Openness Code 1995.

* Our opticians are required to respond to requests for information in relation to the NHS service they provide. You will have a right of access to that information subject to exemptions as contained in the Freedom of Information Act 2000.

* Under the Data Protection Act 1998, you are also entitled to access your own clinical records or any other personal information held about you. Please contact Shore Sight Opticians in order to obtain copies of this information if you wish.

Classes of information

All NHS information at Shore Sight Opticians is held, retained and/or destroyed in accordance with NHS guidelines. Our commitment to publish information excludes any information which can be withheld under the exemptions set out in the Freedom of Information Act 2000 or the NHS Openness Code 1995. Where individual Classes are subject to exemptions, the main reasons are the protection of commercial interests and the protection of personal information under the Data Protection Act 1998. This applies to all Classes in the publication scheme. The information on this scheme is grouped into the following categories:

1. Who we are

Shore Sight Opticians are an independent opticians' practice, established in 2008. We aim to provide an exceptional service to all of our patients, both in taking care of their clinical needs and in the provision of the most appropriate eyewear.

Details of the opticians and optometrists at Shore Sight Opticians can be found in the practice. Please contact Shore Sight Opticians directly to obtain this information. Alternatively details of all UK registered opticians can be obtained from the General Optical Council.

2. Our services

If you are eligible for a free NHS sight test you can visit Shore Sight Opticians for your free test. Shore Sight Opticians will also accept NHS vouchers against purchases of glasses and/or contact lenses. To find out if you are eligible for a free eye test and/or NHS vouchers please contact NHS Direct on 0845 4647 or visit www.nhsdirect.nhs.uk. Alternatively, you can contact your local health authority.

3. Financial and funding information

Shore Sight Opticians will currently receive £20.70 for each NHS sight test performed in the practice. Shore Sight Opticians can inform you of the current cost of an eye exam for those who do not qualify for NHS assistance.

These charges and voucher values are regularly reviewed by the NHS.

4. Information for the public

Information in relation to your eye test, the NHS, family eyecare and safe driving can be obtained in practice.

5. Complaints

A copy of our complaints procedure can be obtained from the practice. If you have a complaint, though, we recommend that you first contact us - on the very rare occasions that a patient is dissatisfied with any aspect of our service, we're always keen to make things right.

6. Our policies and procedures

Details of our Health and Safety policy can be obtained from the practice.

Our privacy policy is very simple. As a patient of our practice, we may hold clinical records for you (including name, date of birth, contact details and clinical information) out of necessity. Any information of this nature that we hold for you will never be passed on to anyone else without your express consent (for example in the case of a referral to your GP).

As a customer of our online store, we need to take your name and address for delivery purposes. Again, these details will never be passed on to anyone else. Payment is handled by PayPal, who hold any payment details - these are not made available to us.

7. This publications scheme

Any changes to the publication scheme and any proposed changes or additions to publications already available will be published in this Class.

Cost of information

For the most part, you will only be charged for printed copies of information required. Some information is available free, but for others there may be a charge. The charges will vary according to how the information is made available. The charges are as follows:

Information accessed via www.shoresight.co.uk or provided by e-mail is free of charge except for charges made by your internet service provider.

Requests for single printed copies are free of charge.

Requests for multiple printed copies or archived copies may attract a small charge to cover incidental costs such as photocopying and postage.

These charges will be reviewed regularly.
